WATER SERVICE CONNECTION ONLINE APPLICATION
1. Download and Print the New Service Connection Application Form.
2. Fill out the blanks with check and Sign the Form.
3. Scan the duly accomplished Application Form and Required Documents (1. One (1) Valid ID, 2. Proof of ownership or right to occupy lot where service is to be connected such as Land Title, Tax Declaration or Deed of Sale 3. Other papers as the situation warrants such as barangay permits, etc.). In case printer and scanner are not available, the applicant may opt to fill-out the fillable PDF format of the New Service Connection Application Form and take photos of signature and other Required Documents.
4. Send the scanned Application Form and Required Documents or the filled-out PDF Application Form along with picture format of signature and other required documents to our customer service e-mail address at pwdcustomerservice1987@gmail.com.
5. Applicant will be notified regarding the status of application within three (3) working days. If the requirements found to be non-compliant, the customer service representative will inform the applicant for the reason and may require to submit other needed documents.
